Required Health Documentation

Thank you to all of you who have submitted the required health documents and completed physical forms for the 2016/2017 school year!

As a reminder:

  • All students entering 9th grade are required to have a current physical examination completed on the approved Illinois Department of Public Health Form. A current physical is defined as having been completed on August 1, 2015 or later.
    There will be an onsite clinic sponsored by Greater Elgin Family Care Center at Jacobs High School on August 3rd, 2016. Appointments are available from 9 am until 4 pm by calling (847)551-8371. Physicals are available for $25, with Medicaid being accepted as well. Immunizations will not be available at this event.
  • All students entering 12th grade must show proof of receiving two doses of meningococcal vaccine (meningitis) unless the first dose was given after the age of 16, then only one dose is required.

Students MUST submit the proper documentation in order to attend classes on the first day.

Incoming 9th grade students are invited to attend Eagle Quest, our Freshman Orientation at Jacobs on August 9.  Click here for more information.

Jacobs families,

The Jacobs High School English Department is dedicated to increasing appreciation, independence, and critical thinking while reading.  Therefore, your child will be responsible for completing a summer reading assignment. The purpose of this assignment is to encourage skill retention and to prepare students for the rigor of the upcoming school year. 

Your child is required to read the novel listed for the English course that he/she will be enrolled in this fall. 

For all classes, your child's comprehension and analysis of the novel will be evaluated through an assignment due on the first day of school.
